I'm using telehealth with an external monitor and I can't hear anything

If you are using a laptop with an external monitor in your setup, you must run the software on the laptop screen in order to hear audio. No audio will be heard if you run the software on an extended display unless the monitor has speakers which can be selected (in the Control Panel of your laptop) as the audio output device.
